Psyllium husk is the best-selling fiber supplement category in the world. It is marketed as a natural appetite suppressant. It has been branded "nature's Ozempic" across thousands of viral videos, recommended by influencers who describe it as a cheap alternative to prescription weight-loss drugs.
In Clark and Slavin's systematic data, psyllium failed to suppress appetite in five of seven treatments. One treatment showed mixed results that trended negative. Only one of the seven showed any meaningful satiety benefit.
For food intake, the results were equally poor. Psyllium reduced how much people actually ate in just one of six tests. Four showed zero effect. One showed mixed or negative results.

Why the Categories on Your Label Don't Predict Anything

Among the soluble viscous fibers tested, beta-glucan and lupin kernel fiber enhanced satiety, but pectin and psyllium had minimal effect. Among fermentable fibers, which theoretically influence appetite through gut bacteria, only beta-glucan consistently worked. Guar gum, inulin, and resistant starch did not.

Which raises a question the data answers in an unexpected way. If the chemical classification does not predict the result, what does?
The Part the Supplement Industry Removed

Feeling Full and Eating Less Are Not the Same Thing

Clark and Slavin compared how fiber treatments performed on two separate measures. First, fullness surveys: how full do you feel? Second, food intake at a later meal: how much did you actually eat? The assumption behind every "I feel satisfied" review of a fiber product is that these two outcomes go together. They don't, 40% of the time.
Of fiber treatments that scored in the top half for making people feel fuller, four out of ten had zero effect on how much those same people actually ate. The fullness was real. The calorie reduction was not.
Even stranger: resistant starch and Fiber One cereal showed the reverse pattern. They did not make people feel fuller on the questionnaires, but they did reduce how much people ate at the next meal. The feeling of fullness and the behavior of eating less were running on different tracks.
The metric the supplement industry measures, the one on the product reviews, the one you use to judge whether your fiber "works," is the metric that fails to predict the outcome you actually care about. Feeling full is one biological signal. Eating less is another. And for nearly half the fiber treatments in this systematic review, only one of them showed up.
If fullness ratings miss what drives actual intake, three other meal characteristics do not. When researchers modelled 2,733 meals in a metabolic ward, energy density, eating rate, and hyper-palatable food percentage predicted 76% of how much people ate — none of which depend on the subjective sensation of being full.
What the Researchers Did Not Measure
Every finding in this review comes from studies lasting 24 hours or less. The researchers measured what happened to appetite and food intake on the same day someone consumed fiber, not what happened over weeks or months.
This matters because some fiber types, particularly fermentable fibers, may need chronic exposure to influence appetite. The mechanism would run through your gut bacteria producing short-chain fatty acids that affect appetite hormones over time, a pathway that a single-day study cannot detect.
The researchers noted this directly: "weeks or months may be needed to detect changes."
If you have been taking psyllium for months and genuinely feel less hungry, this review was not measuring your experience. It was measuring the first 24 hours. That is a real limitation, and it is worth knowing alongside the failure rates.
There is another honest nuance. The relationship between fiber and body weight in population-level studies is real, but the researchers suggested it might not operate through appetite at all.
Eating less during the meal itself, or changes in how your body stores fat, could explain the pattern. Higher fiber intake tracks with lower body weight even when most fiber types do not suppress hunger between meals.
A second systematic review by Wanders and colleagues, covering 104 randomized controlled trials, found similar patterns. More viscous fibers reduced appetite in 59% of comparisons versus 14% for less viscous types.
The fiber-type specificity held across appetite, energy intake, and body weight outcomes. The evidence from two independent systematic reviews points in the same direction: not all fiber works, and type determines everything.
The Shopping List Revision

No children, pregnant women, or older adults with age-related health conditions were included. The review did not separate results by sex — whether fiber affects appetite differently in men and women remains unknown from this data.
No formal quality assessment was applied to the 44 included studies — the researchers did not use standard tools to evaluate how well each trial was designed. Study power, blinding quality, and randomization methods varied widely.
The classification system treats every study equally regardless of sample size — a study with 12 participants carries the same weight as one with 200. The search was limited to one database (PubMed) and English-language publications only.
Strong enough to trust the fiber-type ranking, honest enough to note the method. A systematic review of 44 studies covering 38 fiber types is a substantial evidence base — nobody else has compiled this comparison.
But the analysis used narrative synthesis rather than statistical pooling. There is no single number summarizing how large the appetite effect was across studies, no range showing how precise that number would be, and no formal measure of how much the individual studies disagreed with each other.
