The answer is yes — creatine does work differently in a plant-based body. But it is not the across-the-board win the supplement aisle sells you. The edge is real for one outcome, promising for a second, and shot down for a third. Knowing which is which changes what every scoop should mean to you.

If your muscles absorb twice as much creatine, does that turn into bigger gains in the gym?
One trial tested exactly that — creatine plus eight weeks of weight training, measuring body changes head to head. The vegetarian creatine group gained 2.4 kilograms of lean mass compared to 1.9 for meat-eaters. Their muscle endurance improved roughly three times more.
That half-kilogram gap over two months is the kind of change that shows up slowly — when something in your build shifts and someone else notices before you do.
That is one study. Forty-two people.
It is the only trial in Kaviani’s review that combined creatine with weight training — the exact setup most readers care about. Burke's findings are promising. They are not settled.
Where the story breaks
The review looked at nine studies and concluded the vegetarian edge is "not conclusive." Different studies measured different things. Endurance and body changes showed the clearest signal. Power output was equal. Aerobic fitness showed no diet-specific benefit.
Having more fuel in the tank does not mean you win every race. The fuel edge is real. Whether it changes the finish line depends on which race you measure.
And then there is the brain.
Brain creatine is where the pattern breaks entirely. Brain creatine starts at the same level in vegetarians and meat-eaters. Taking creatine does not change brain levels. There is no clear reason for the brain boost two earlier studies reported.
Then a newer study — 123 people, half vegetarian, preregistered and double-blind — found nothing. No diet-specific effect on any brain measure.
Europe's food safety body reviewed 21 studies on creatine and the brain. They rejected the health claim. The only short-term effect showed up at 20 grams per day — four times the normal dose.
The part of the creatine story that went most viral is the part that has fallen apart since.

If you recently stopped eating meat, even one gram daily kept stores from dropping in one trial. The amount needed to maintain is lower than most guides say.
One question usually follows this one. Am I taking creatine because my protein is failing — or because creatine adds something different on top of protein that already works?
